Description Purpose of Position:
The Qualified Professional provides support to the Lead Qualified Professional in overseeing the development and implementation of strategies and structures in Residential/Day Support facilities to assist persons served so they may achieve their personal goals.

Responsibilities:
Provide coverage in facilities for understaffed shifts as needed Ensure individual's Progress summaries are correct and submitted prior to deadlinePrepare annual paperwork for ISP meetings (Short Range Goals forms)Provide monthly inspection schedule to complete unannounced visits of facilities/vans.
Collect all Documentation from facilities and Review Progress Notes, Supervised/Group Notes and Incident reports for correction (ensure managers cc Lead QP on all reports)Ensure ALL incident reports are correct and organized for the quarterly reports to be completed by the end of each month.
Print and review Alliance weekly updates.
Serve on the Interview Panel for vacancies and provide feedback of candidates to QPParticipate in all required team meetings, committees, and audits as requiredComplete agenda for QA/QI meetings and various meetings as neededConsult with QP on appropriate solutions to complex issuesPerform other duties as defined by the QPService Management:
Provide staff training for Orientation and Documentation, individual disabilities and Person-Centered Plan (PCP) implementation.
Assist QP with clinical and technical assistance for support staff, as neededAssist QP with implementation of the PCP for clinical appropriateness of service delivery.
Quality Assurance and Improvement:
Ensure consistent provision of quality services to consumersMonitor the implementation of the PCP to assure that services are being provided in a manner that is professional, effective, and consistent with governing rules, statues, service definitions and the PCP.
Ensure the case management, supervision, and service notes are being completed in a manner consistent with Federal, State, and Agencies statues, rules, policies and procedures respectively.
Submit reviewed and signed service data to the Medical Records Manager for billing verification, filing and submission to the Home Office, as applicable.
Participate in peer review activities of agency offices as requested.
Communicate ideas on improving services to his/her supervisor.
CommunicationMaintain consistent communication with his/her supervisor regarding documentation, authorization, consumer progress toward goals, and consumer satisfaction.
Maintain regular contact with persons served, their treatment teams, guardian, Office Manager and significant others in persons lives.
Qualifications:
Education/ExperienceThe minimum requirement for this position is either:
A master's degree in a related field and 1 year of supervised, postgraduate, experience servingpersons with developmental disabilities.
A bachelor's in Human Service related field with 2 years of supervised, post graduate,experience serving persons with developmental disabilities.
A bachelor's degree in another field with four years for supervised, postgraduate, experienceserving persons with developmental disabilities.
Must have knowledge and experience with the age and population served and meet QualifiedMust have IDD experienceProfessional Status and defined by 10A NCAC 27G.

The Qualified Professional Assistant is responsible for insuring systems are in place to address the on-going facilitation of services to each consumer, as well as oversees the collective services of his/her caseload , which has the potential to severely affect the health and safety of consumers and staff.
This position has a fiduciary responsibility to Agency including but not limited to billing, in which errors could result in significant funding infractions for the agency.
Supervision and Training Provided to Employee:
The local office will provide initial training.
On-going training and supervision is provided by the supervisor.
Physical Effort:
A variety of physical activities are necessary for this position and depending upon the needs of the consumers of the agency.
From time to time travel will be required, which may include out-of-town travel.
There will also be interaction with consumers served at each local office and facility.
Work Environment and Conditions:
The Qualified Professional Assistant works primarily in environmentally controlled offices where conditions are pleasant, although visits to the sites where services are being provided are necessary in order to ensure the quality and provision of services to the consumers.
